If you want to take the survey and get yourself a free sandwich, you need to have a computer with access to the Internet as well as a Chick-fil-A receipt because you need to enter the serial number printed on it to proceed with the survey.
Here are the steps:

- Enter the serial number found on your receipt.
- Click the Start button.
- Begin answering the survey questions. Make sure to answer them as honestly and as detailed as possible.
- Once you have completed the survey, you will be given a validation code which you need to write on your Chick-fil-A receipt and claim it on the date indicated otherwise it will be rendered invalid.
